The surgery provides holistic care and support to patients experiencing short or long term mental health challenges. We work closely with the local specialist mental health services and run regular clinics. Our approach is always to consider both mental and physical health and to provide care for the whole person.
Substance misuse
The surgery provides holistic care for people with a drug or alcohol dependency and we work closely with the local drug and alcohol services. There is a weekly clinic run by a drug worker who provides ongoing support.
Comprehensive frailty service
We have a comprehensive frailty service at the surgery run by a dedicated team. Frailty is a term used to describe anyone who has multiple needs, often including health and social care, for which an all-encompassing and coordinated approach is going to be more effective.
The includes a doctor with a special interest, a dedicate care manager, a social prescriber, and pharmacy and nursing input. The team liaises very closely with other NHS services, including district nursing, and social services, and also has support from a named hospital consultant specialising in frailty.
The team supports people living at home, alone or with family, and also people living in care homes and sheltered accommodation.
Learning disability service
We are experienced at supporting people with a variety of learning difficulties, and their carers, and this includes a comprehensive proactive annual health check in addition to any routine requests.

9. Access new treatments through our research hub
All local patients can access our research hub based at Richford Gate Medical Centre which is run by our doctors and nurses. Ask your GP if you’re interested. Our recent/current studies have covered hypertension, lipid management, treatments for covid, and a home monitoring programme for frail patients living alone. All of these studies help the NHS improve care and find new treatments
6. Minor surgery clinic
We have a minor surgery service based at North End Medical Centre. From minor skin lesion removal to pain and steroid injections, we can treat patients quickly so they don’t have wait for hospital treatment
7. Respiratory hub
We run a respiratory health hub at North End Medical Centre which is accessible to all residents in Hammersmith & Fulham, whichever surgery you’re registered at in the borough. The service diagnoses and monitors people with respiratory problems such as asthma and COPD. The staff are specially trained and are supported by the respiratory team at Charing Cross Hospital
5. Diabetes
We run specialist clinics for people with diabetes.
Regular monitoring of blood sugar levels, cholesterol, weight, and blood pressure, have a really positive effect on minimising the long term effects of diabetes. Our clinical team also provides advice about diet and lifestyle and can offer motivational support. The doctors are supported by two health coaches that run group and individual sessions to help you understand what you can do to improve your health and mood.
